VMware provides Health Insurance through a Group Insurance Plan with Medicover Sp. z.o.o.
All regular employees and their spouse/partner and eligible child dependents. Children are eligible up to 18 years of age and up to 25 years of age if they are full-time students. Employees can also purchase a cover for their or their spouse’s parents up to 75 and up to 85 years of age.
The Group Medical Insurance plan provides healthcare coverage incorporating the following benefits:
- 24-hour medical hotline and ambulance
- Diagnostics/test and examinations, all available within own medical centres
- Unlimited specialist consultations (full range, each specialization)
- Pregnancy care program
- Flu vaccinations and coordination of vaccinations for children
- Psychotherapy and rehabilitation services
- Allergy tests
- Psychological, psychiatric and dietician consultations
- Mandatory health checks
- First aid training
- Unlimited home visits
- 24-hour emergency dental care
- 15% discount on medical dental treatment and dental prosthetics
- Dental check-up and cleaning once a year
Within the first two days of employment, please complete the Application form, which can be found in the Resources section, and email it back to the HR Specialist. You will find the Specialist’s email address in your Welcome email. Then drop a letter containing the hard copy of the form to Reception who will send it to the HR Specialist for you.
VMware pays 100% of the premium costs towards the costs of your cover for the Blue Package. You can upgrade to a Gold or Senior Level plan if you wish. In this case, the additional cost will be at your expense and will be collected from you through monthly salary deductions.
When Coverage Starts
VMware offer coverage from your first day of employment, however the actual start date will depend on when you submit your Application form. If you submit it before or within the first couple of days of your employment, your cover will start from your first day.
After that, you can submit your Application form by 20th day each month. Your cover will then become active from the 1st day of the next month, following the month in which you had submitted your declaration.
Please note that if you submit your Application form within your first two days of employment, it may take up to a week for your cover to become active on Medicover system. Should you need an emergency appointment during this period, please request an invoice during your appointment and raise a HelpNow ticket, attaching the invoice.
When Coverage Ends
Coverage ends on your last day of employment with VMware.
Changing Cover Level or adding dependents during Employment
If you already have an active package and wish to change it or to add or remove a dependant, please complete a new Application form and email and post it to the HR Specialist by the 20th day of each month. The new cover level will become active from the 1st day of the next month.
Benefit in Kind Tax and Salary Deductions
The premiums paid by VMware are subject to income tax, therefore you will pay tax on the premiums in accordance with Polish tax legislation.
Enhancements to Gold and Senior packages will be processed by Payroll on a monthly basis through salary deductions.
