VMware provides a health insurance through Maxicare Healthcare Corporation.

Eligibility

All regular full-time employees and their eligible family members may enroll in health insurance. Eligible family members include:

If you are married: (1) Legitimated spouse under 65 years old; (2) Unmarried legitimate, legitimated, adopted children residing with the Principal member who are at least 15 days old up to 26 years old, not gainfully employed and have no children of their own.
If you are single: Parents under 65 years old OR domestic partner with 18 to 65 years old, including same sex partners.
If you are single parent (including widow/widower): (1) Unmarried legitimate, legitimated, adopted children residing with the Principal member who are at least 15 days old up to 26 years old, not gainfully employed and have no children of their own; (2) Domestic partner with 18 to 65 years old, including same sex partners.

Out-Patient Benefits

All outpatient consultations and procedures, as long as medically necessary, covered up to 500,000 PHP

Optical Assistance Program – reimbursement basis

Prescribed Eyeglasses (Lenses and Frames), and contact lenses, covered up to 10,000 PHP per person

Outpatient Medicines – reimbursement basis

Prescribed Outpatient Medicines including Take Home Medicines, covered up to 25,000 PHP per family

Inpatient Care

Room and board accommodation, cover Platinum (Open Private) up to 500,000 PHP
All other items directly related in the medical management of the patient, covered up to 500,000 PHP

*Out of network availment for IP and OP: Reimbursable up to 100.00% of hospital bill and professional fee based on Maxicare rate

*Pandemic coverage: Cover Infectious diseases that are declared epidemic or pandemic by the Department of Health, World Health Organization or any recognized health authority up to 500,000 PHP
