Medical

VMware provides a health insurance through AXA China Region Insurance Company Ltd.

Eligibility

All regular full-time employees and their eligible family members may enroll in health insurance. Eligible family members include:

• Legal spouse under 70 years of age
• Domestic partner under 70 years of age (Need submit Declaration of Committed Relationship to HR Source)
• Child(ren) 14 days afterbirth until their 18^th birthday or until their 23^rd birthday if they are full-time students

Basic Hospitalization (In-patient)

• Intensive Care Unit up to HK $2,400 per day (max. 10 days per disability)
• Room & Board up to HK $1,500 per day  (max. 60 days per disability)
• In-hospital doctor visit up to HK $1,500 per day (max. 60 days per disability)
• Hospital expenses up to HK $26,000 per disability
• Surgeon fees limits per disability:
  • Complex Operation: HK $69,000
  • Major Operation: HK $46,000
  • Intermediate Operation: HK $23,000
  • Minor Operation: HK $9,200
• In-hospital specialist consultation up to HK $5,000 per disability
• Home health care up to HK $45,000 per disability

• Hospital Cash for HA confinement up to HK $750 per day (max. 60 days per disability)

• Benefits are subject to an overall maximum of HK $394,000 per disability.

Supplementary Major Medical

• 80% reimbursement up to an annual maximum of HK $200,000 per disability (HK $500 deductible applies).

Maternity

• 100% reimbursement up to the following maximums:
• Miscarriage: HK $12,500
• Normal delivery: HK $22,000
• Cesarean: HK $33,000

Clinical (Outpatient)

• 100% reimbursement up to the following maximums:
• Doctor Consultation up to HK $450 per visit (max. 35 visits/year)
• Chinese Herbalist/Bonesetter/Acupuncture up to HK $350 per visit (max. 15 visits/year)
• Routine Check-up up to HK $1,500 without visit limitation, cover “Vaccinations and Immunizations”
• Specialist Consultation up to HK $750 per visit (max 10 visits/year)
• X-Ray and Laboratory Tests up to HK $1,800 per disability (requires written referral from attending physician)
• Prescribed medicines and drugs: HK $2,200 per year (requires written referral from attending physician)
• Physiotherapy & Chiropractor up to HK $400 per visit (max 35 visits/year)

*Please check the AXA App for your claim history, in case you cross annual visit limitation, you are liable to pay the insurer directly for the excess visit.

Dental

• Covers routine treatments up to a daily maximum of HK $1,000 and an annual maximum of HK $5,500.

General Exclusions

There are pre-existing conditions which may be excluded under the Plan. You will need to review the AXA Insurance Program Scheme ^[1] for the relevant details.

VMware Contributions

VMware pays 100% of the premium costs for you and your eligible family members.

When Coverage Begins

Coverage for you and your eligible family members begin on your first day of active work as a VMware employee, provided you Enroll ^[2] within 30 calendar days of your start date.

When Coverage Ends

Coverage for you and your eligible family members will end on the last day of your employment with VMware.
