VMware provides Vision benefits through the Health Insurance [1] plan with Malakoff Humanis (MH).
Eligibility
All regular employees and their qualified dependents. Participation in the Health Insurance plan is mandatory.
Benefits
The Health Insurance [1] plan provides the following Vision benefits:
- Lenses, Contact Lenses: 90% of actual costs
- Frames: € 100
- Contact Lenses (includes disposables): 8% of MSSC
- Laser Eye Surgery: € 600 per eye
Pre-Existing Conditions
Pre-existing conditions are excluded under the plan.
Enrolment
To enrol [2], you must complete and return the Health Insurance enrolment form sent to you by HR Source [3] [VMware network access required] within 30 days of your start date.
Contributions
The Vision benefits are provided through the Health Insurance [1] plan. VMware pays 60% of the premium cost for the Health Insurance [1] for you and your eligible dependents.
Coverage Begins
Coverage for you and your dependents under the Health Insurance [1] plan will begin on your first day of active work as a VMware employee, provided you enrol [2] within 30 days of your start date.
Coverage Ends
Coverage for you and your dependents under the Health Insurance plan, will end on the last day of your employment with VMware.
