2019 Flexible Spending Accounts Claim Submission Reminder
All claims incurred in 2019 must be submitted to TRI-AD [VMware network access required] by March 31, 2020 for processing.
2019 Rollover – The General Purpose and Limited Purpose FSA plans offer a rollover option up to the IRS limit of $500. Any 2019 balance over $500 remaining after March 31, 2020 will be forfeited on April 1, 2020 due to IRS regulations. Please note, the Dependent Care FSA plan does not offer a rollover and any remaining balance after March 31, 2020 will also be forfeited on April 1, 2020.
Your medical election from your 2020 Open Enrollment impacts what type of Flexible Spending Account your Carryover FSA money was rolled into for 2020:
- If you elected the UHC Traditional PPO plan or the Kaiser HMO N. CA plan, your rollover amount was placed in a General Purpose Flexible Spending Account (GPFSA).
- If you elected the UHC HSA PPO plan, your rollover amount was placed in a Limited Purpose Flexible Spending Account (LPFSA).
You may check your 2019 account balances with TRI-AD [VMware network access required] or by calling Customer Service at 1-888-844-1372, Option 2, Monday through Friday 5:00 am to 6:00 pm PT.
Form 1095 and ACA Required Dependent Information
Federal reporting requires VMware to have every dependent’s full legal name, date of birth, and Social Security Number on file. Please review both you and your dependent’s information within Workday [VMware network access required]. Without this dependent information, VMware will not be able to accurately report coverage for you and your covered dependents as required under the Affordable Care Act (ACA).
To review your dependent information, sign into Workday [VMware network access required] and click the ‘Benefits’ icon from the homepage. Under ‘Change’ select ‘Dependents.’ Click ‘Edit’ next to the dependent whose details you would like to review/change. Make any necessary changes and click ‘Submit.’ If you do not have any changes to make after reviewing click, ‘Cancel.’
You will receive your Form 1095 on or before the IRS deadline of March 2, 2020.

As part of VMware’s commitment to a green environment we are requesting that you opt-in to receive your Form 1095 electronically only, rather than via mail and electronically. To opt-in, login to Workday [VMware network access required] and click the ‘Benefits’ icon from the homepage. Under ‘Change’ select ‘1095-C Printing Election’. Select ‘Receive electronic copy of 1095-C’ and click ‘Ok’ and ‘Done’. The deadline for opting-in is Friday, January 10, 2020.
Now is the perfect time to review your beneficiary designations for your benefit plans to ensure they are up to date. Update your HealthEquity (HSA), Fidelity (401(k) and Non-Qualified Deferred Compensation Plan) and E*TRADE (stock) accounts via the providers website.
You may update your Basic Life, Supplemental Life and AD&D beneficiaries via Workday [VMware network access required].

Confirm Your Personal Information & Emergency Contacts
VMware has requirements to mail important documents to your home. Take a moment to ensure your home address is correct Workday [VMware network access required]. For assistance on updating your personal information, review the Change My Contact Information [VMware network access required] quick reference guide. Please note, any changes will be sent to all VMware benefit vendors (except E*TRADE) 1-2 weeks after you have submitted the update.
To update you address within E*TRADE, log in to your account, hover your mouse over the Accounts title on the top left of your screen and select My Profile, select View/Edit next to Mailing Addresses.
Qualified Life Events
If you experience a qualified life event (marriage/domestic partnership, gain/loss coverage, etc.) you have 30 calendar days from the date of the event to report it and make changes to your benefits coverage. For a birth/adoption event you have 60 calendar days from the date of the event to report it and make changes to your benefits coverage.
