Medical

As a VMware employee, will be provided with private Medical Insurance through Omint.

Eligibility

All regular full-time employees and their qualified dependents are eligible. Qualified dependents include spouse, domestic partner, children, step-children until 21 years old, and students until 24 years old.

Network

Participants can access care through the Omint Network or can request reimbursement medical care provided outside the Omint Network.

In order to receive reimbursement for medical care received outside the Omint Network you will need to submit your expenses to Omint for reimbursement. Check out the Reimbursement Guide for more details.

Omint offers comprehensive medical coverage including the following services:

• Outpatient Coverage
• Emergency care
• Physicians
• Exams: radio-logical and laboratory;
• Physical therapy
• Speech therapy
• Acupuncture
• Psychiatry
• Vaccines: as adopted by the Ministry of Health;
• Organ Transplants
• Hospitalizations and surgical clinics;
• Small surgical events held outside the hospital;
• Obstetrics, including prenatal care and hospitalization

Enrollment

If you wish to enroll in the VMware medical plan, you must do so within 30 days of your start date. Coverage is effective on your date of hire. You can enroll in the Omint medical plan through MyLife [VMware network access required] during your first 30 days.  It can take up to 5 business days from the time you complete your enrollment for VMware to receive your unique Omint ID Number.

If you experience a qualified life event and want to make changes to your enrollment will need to report the life event via MyLife [VMware network access required].

Pre-existing Conditions

Pre-existing conditions may be excluded under the plan.

Contributions

VMware will pay 100% of the premium costs for you and your eligible dependents.

When Coverage Begins

Coverage for you and your dependents will begin on your first day of active work as a VMware employee, provided you enroll within 30 calendar days of your start date.

When Coverage Ends

Coverage for you and your dependents under the VMware Medical plan will end on the last day of the month in which your employment with VMware ends.
